Abstract
Pipeline incidents are very often caused by so called third party activities. Often these are maintenance activities conducted by contractors. The traditional quantitative risk approaches show shortcomings in modeling organizational or inter-organizational impacts. Therefore, an integrated approach to risk and safety is needed including human, organizational and inter-organizational factors. The paper will give examples of a more holistic risk approach using FMEA to model the human and organizational influences on the reliability of non-destructive-testing. Furthermore, an example of an in depth-incident analysis with focus on inter-organizational aspects will be presented. A combination of an integrated risk assessment and an incident analysis based organizational learning will help to improve the organizational resilience.
